- Partnering for Adams County Kids (PACK) Referral
- Partnering for Adams County Kids (PACK) is a 2Gen approach and multi-system integrated planning program for Adams County children, youth, and families with children and/or youth in the home from birth to 21 years of age. PACK is a collaborative meeting that includes children/youth, parent/guardian/caregivers, and representatives from community agencies in Adams County who support youth and families. The goal is to produce an action plan that will connect the family with vital resources to improve outcomes and increase service connection for Adams County families. This program serves Adams County residents with a child in the home from birth to 21 years.
Upcoming Meetings
The Adams County Collaborative Management Program (ACCMP) meets bi-monthly, on the second Tuesday of the month from 2 – 4 p.m. at the Adams County Human Services Center.
The Adams County Collaborative Management Program (ACCMP) Executive Committee meets monthly, on the first Wednesday from 12 pm-1:30 pm, virtually.

Pilot Family Engagement Program to enhance supportive services for families to prevent truancy court related processes.
Almost Home
Funding for the family shelter and move in assistance to support housing stability for families transitioning out of homelessness.
Five Star Education Foundation
Zerobarrier resources and navigation services for Adams 12 families, aimed at reducing access barriers for essential supports and enrollment in public benefits programs.
Servicios de la Raza – VISTAS Program
Zero barrier resources and navigation assistance for families experiencing intimate partner violence and seeking culturally responsive support services.
Almost Home
Provides 100% coverage of move‑in costs for family shelter residents to support stable housing transitions.
Commerce City Community Well-Being
Partnership between the Conflict Center and Adams 14. Violence prevention program focused on community safety, youth support, and early intervention strategies.
Funding Regulations & Agreements
- Section VI of the CMP MOU “Utilization of CMP Funds” states, “ensure that CMP funds are being used to serve children, youth, and families that are involved in multiple systems or at risk of involvement in multiple systems. This includes funds being used to serve children, youth, and families who are part of an ISST, CMP prevention program, or as a mechanism to increase collaboration among Parties.”
- 12 CCR 2509-4-7.303.33.B.2. states “CMP prevention programs must demonstrate a multi-systemic approach. Programs must demonstrate in the MOU that multiple disciplines were involved in the development or enhancement of the program or that multiple agencies are involved in the delivery of the service.”
